Registration for VPA & ESOL Students

WELCOME VPA PARENTS!

We would first like to thank you for accepting your seat into our amazing Visual and Performing Arts Program. We look forward to an amazing year with your Scholars! Below you will find all of the documents that are needed to update your residency for the upcoming 2023-2024 School Year.

Enrollment Information

  • PARENT(S) STATE ISSUED I.D.: 

(A Photo must be included – Example: Driver’s License, Passport, Or Permanent Residency Cards will be accepted) 

  • BIRTH CERTIFICATE OF REGISTERING STUDENT: 

(The Birth certificate must reflect the exact same name as shown on the registering parents I.D/License. If it does not, the Parent must provide a copy of the marriage license or proper documents reflecting the name change. 

*If you are NOT listed as a parent on the student's Birth Certificate, you will need a court order signed by a judge stating guardianship. 

*To the Mothers, If you’ve been married and your name shows different on the students birth certificate then on your ID, please include a copy of your marriage license. 

  • PRIMARY PROOF OF RESIDENCY: 

-Deed of Property 

-Settlement Statement including the signature page 

-Lease Agreement including the signature page 

(If the lease is not current (Within the year), a Signed Residency Verification form from the leasing office is needed), If the lease is personal it MUST be notarized. 

-Mortgage Statement 

(Must be dated within the current month of registration) 

-Property Tax Bill (Must be within the current year) 

(These documents must be in the name of the registering parent) 

  • PROOF OF MAIL: 

-1 (One) Utility Bill: ex. (Gas, Water, or Electricity) 

OR 

2 (Two) Pieces of Mail, Ex.: Bank Statement, Paystub, Home Cable Bill, Home 

Telephone Bill, Monthly Insurance Statement, Mail from a Government Agency, Monthly Medical statement, Monthly Car Statement. NO CELL PHONE BILLS OR W2s WILL 

BE ACCEPTED! 

All Mail must include the full name of the registering Parent, full address and must be dated for the current month of registration or 1 month prior. For Example, if you are registering your student in the month of August, your mail should be dated for August or July.)

We would first like to Welcome you to the Wildcat Family! We look forward to an amazing year with your Scholars! Below you will find all of the documents that are needed for you to complete your students’ enrollment.

Enrollment Information

FROM THE PARENT:

  • PARENT(S) STATE ISSUED I.D:

SHARED HOUSING DISCLOSURE

Some families will not have a house or apartment of their own, but will live with a relative or friend; therefore, a notarized Shared Housing Disclosure Form, signed by both parties detailing the living arrangement, is required. If you are paying rent to someone, it is NOT considered Shared Housing.

The following conditions also must be met:

  • All parties must sign the notarized Shared Housing Disclosure
  • The relative or friend, with whom the parents reside, must show their proof of residence.(Current Mortgage Statement, Property Tax Bill, Deed, Settlement Papers with Signatures, Lease Agreement with Signatures.
  • The parent registering the child must present official mail to the school within 30 days of enrollment ONLY IF the parent has moved in less than 30 days prior to registration. If the parent has lived at the current address for more than 30 days, 2 pieces of mail are required at the time of registration.  The parent must provide items such as a statement from an employer showing the new address, bank statements, car registration, car insurance Statement, government or official correspondence, Medical bill etc.
  • The parent should be advised that the school system reserves the right to monitor this type of proof periodically or as needed.

(This Packet should include Grades, Transcripts, Immunizations, and a copy of the students Birth Certificate)

  • -ISCO REGISTRATION PACKET- ____________

(This packet was given during the pre-registration process at the International Student Counseling Office)

  • -COMPLETE IMMUNIZATION RECORD -__________

(ALL Immunization records MUST be checked AND cleared by Northwestern's School Nurse Mrs. D. Yon BEFORE registration can be completed)

NOTE: If the Student is coming from another County School (not PG County), a Physical is needed if the student is interested in playing a sport.

¡BIENVENIDOS PADRES Y ESTUDIANTES!

¡Esperamos tener un año increíble con sus estudiantes! A continuación encontrará todos los documentos necesarios para completar la inscripción de su estudiante

  • IDENTIFICACIÓN EMITIDA POR EL ESTADO DEL PADRE(S):

(Se debe incluir una foto. Ejemplo: se aceptarán la licencia de conducir, el pasaporte o las tarjetas de residencia permanente)

  • ACTA DE NACIMIENTO DEL ESTUDIANTE DE REGISTRO:

(El certificado de nacimiento debe reflejar exactamente el mismo nombre que se muestra en la identificación/licencia de los padres registrados. Si no es así, el padre debe proporcionar una copia de la licencia de matrimonio o los documentos correspondientes que reflejen el cambio de nombre.

 

*Si NO figura como padre en el certificado de nacimiento del estudiante, necesitará una orden judicial firmada por un juez que establezca la tutela.

*A las madres, si han estado casadas y su nombre aparece de manera diferente en el certificado de nacimiento de los estudiantes que en su identificación, incluya una copia de su licencia de matrimonio.

  • PRUEBA PRINCIPAL DE RESIDENCIA:

-Escritura de Propiedad

-Declaración de liquidación que incluye la página de la firma

-Contrato de arrendamiento incluyendo la página de firma

 (Si el contrato de arrendamiento no está actualizado (dentro del año), se necesita un formulario de verificación de residencia firmado de la oficina de arrendamiento), si el contrato de arrendamiento es personal, DEBE ser notariado.

-Estado hipotecario

 (Debe estar fechado dentro del mes actual de registro)

-Factura de impuesto predial (debe ser del año en curso)

(Estos documentos deben estar a nombre del padre que registra)

  • PRUEBA DE CORREO:

-1 (Uno) Factura de servicios públicos: ej. (Gas, Agua o Electricidad)

O

2 (Dos) Piezas de Correo, Ej.: Extracto Bancario, Recibo de Pago, Factura de Cable de Casa, Factura de Teléfono de Casa, Estado de Cuenta Mensual de Seguro, Correo de una Agencia Gubernamental, Estado de Cuenta Mensual Médico, Estado de Cuenta Mensual de Auto. ¡NO SE ACEPTARÁN FACTURAS DE CELULAR NI W2!

Todo el correo debe incluir el nombre completo del padre que registra, la dirección completa y debe tener la fecha del mes actual de registro o 1 mes anterior. Por ejemplo, si está inscribiendo a su estudiante en el mes de agosto, su correo debe tener la fecha de agosto o julio).

DIVULGACIÓN DE VIVIENDA COMPARTIDA

Algunas familias no tendrán una casa o apartamento propio, sino que vivirán con un pariente o amigo; por lo tanto, se requiere una Declaración de Vivienda Compartida notariada, firmada por ambas partes que detalle el arreglo de vivienda. Si le está pagando el alquiler a alguien, NO se considera Vivienda Compartida.

También se deben cumplir las siguientes condiciones:

  • Todas las partes deben firmar la Divulgación de Vivienda Compartida notariada.
  • El pariente o amigo, con quien residen los padres, debe mostrar su prueba de residencia (Declaración de hipoteca actual, factura de impuestos sobre la propiedad, escritura, documentos de liquidación con firmas, contrato de arrendamiento con firmas)
  • El padre que inscribe al niño debe presentar el correo oficial a la escuela dentro de los 30 días posteriores a la inscripción SOLO SI el padre se ha mudado en menos de 30 días, se requieren 2 piezas de correo en el momento de la inscripción. El padre debe proporcionar elementos como un estado de cuenta de un empleador que muestre la nueva dirección, estados de cuenta bancarios, estado de cuenta del seguro del automóvil, correspondencia gubernamental u oficial, factura médica, etc.
  • Se debe informar a los padres que el sistema escolar se reserva el derecho de monitorear este tipo de prueba periódicamente o según sea necesario.

DE LA ESCUELA ANTERIOR

Este paquete debe incluir calificaciones, expedientes académicos, vacunas y una copia del certificado de nacimiento del estudiante.

  • PAQUETE DE REGISTRO ISCO

Este paquete se entregó durante el proceso de preinscripción en la Oficina de Consejería para Estudiantes Internacionales

  • REGISTRO DE INMUNIZACIÓN COMPLETO

TODOS los registros de vacunas DEBEN ser revisados ​​Y aprobados por la enfermera escolar de Northwestern, la enfermera Yon ANTES de que se pueda completar el registro

NOTA: Si el estudiante viene de otra escuela del condado (no del condado de PG), se necesita un examen físico si el estudiante está interesado en practicar un deporte.
