Please bring the following documents at the time of registration:
- Proof of birth date -
preferably the child's birth certificate. The child’s name,
child’s birth date and parent’s name must be listed on the
document provided.
- Photo ID
- Legal proof of residence
- settlement papers, rental contract, etc. (Photo
ID is NOT acceptable proof of residence)
- Student transfer card
- or last report card from previous school, for students
entering from another school system. Under state law, an
"Affidavit of Disclosure" must be completed at the time of
registration. This form is a verification of residence for newly
arrived or initial enrollments. Anyone found guilty of giving
false information on this form may be fined as much as $250 in
penalties.
- Record of immunization

The Maryland State Department of Health and Mental Hygiene has
regulations which require certain immunizations before children
can be admitted to school. Required immunizations are
diphtheria, pertussis (whooping cough – for children through age
6), tetanus, polio, rubeola (measles), rubella, mumps, and for
the 2003-2004 school year, children in preschool through grade
two must have varicella (chicken pox) and hepatitis B vaccines.
Regulations:
(1) Require two doses of rubeola (measles) vaccine for
kindergarten students, with both doses given on or after the
first birthday, and separated by at least 30 days; one dose is
required for pre-kindergarten students;
(2) Require one dose of mumps, rubella and varicella vaccines
given on or after the first birthday. Physician documented
history of chicken pox disease is acceptable in lieu of
vaccination;
(3) Require Haemophilus influenza, type B (HIB) vaccine for
educational preschool programs;
(4) Recommend a physical examination of each child entering the
public school system for the first time. The examination must be
completed within the period of nine months before school entry
or up to six months after entry. The examination must be
performed by a physician or certified nurse practitioner.
At the beginning of the 2003-2004 school
year, the parent or guardian of a child who currently resides, or
has previously resided, in an at-risk area for lead poisoning shall
provide to the designated administrator of the child’s school or
program, evidence that the child has had blood lead testing, on
entry into a Maryland public pre-kindergarten program or Maryland
public school system at the level of pre-kindergarten, kindergarten
or first grade. A form approved by DHMH will be provided for
documentation of the blood testing for lead poisoning.

AGE REQUIREMENTS:
The Maryland State Board of Education
adopted recommendations by the Maryland Kindergarten Committee
on December 4-5, 2001, which include changing the entrance day
for schooling from December 31 to September 1. As a result, the
State Board adopted the required changes in COMAR, May 23, 2002.
As a result of this adoption, children will have to be at least
five years of age by September 1 before being allowed to start
kindergarten. This change will be phased in until school year
2006-07. The plan began changing the eligibility dates for
children entering pre-kindergarten for the 2002-03 school year.

pre-kindergarten, kindergarten and first grade students.

STUDENT TRANSFERS (Policy
5110.3)
Under limited circumstances as provided
by Board of Education policy, individual students may be granted
transfers from the assigned school to another county school.
Copies of the brochure, Student Transfers, which includes the
Board of Education Student Transfer Policy, are available from
each school principal or through the Students Transfers Office,
which also evaluates and processes transfer requests.
