1. Proof of Residency (Must be Deed, Lease, Settlement papers, Rental
Agreement, or Most Current Property Tax Bill) Notarized statement if you
are living with someone.
2. Transcript & W/D records from previous school
3. Birth Certificate
4. Shot Record
5. Social Security Card
