2003 - 2007 Graduates Transcript Request Form Microsoft Word Version (save and complete) \| Acrobat Reader Version (print and complete) Send or Bring the Completed Form to the LHS Guidance Department, or Email the request to Ms. Swartz, Guidance Secretary. There is a $3.00 charge for each transcript.

Current Students
Fill out a Transcript Release Form. Parent must sign if student is under 18. Current students receive 5 transcripts at no cost, additional transcripts are $3.00 each. Release forms available in the Guidance Department or Online
