Required Documents at Time of Registration

 Proof of Residency (Must be Deed, Lease, Settlement papers, Rental Agreement,
or Most Current Property Tax Bill)

bullet

and a current bill (electric bill, telephone bill, or cable bill)

bullet

Notarized statement if you are living with someone, and proof of the person's residency
as stated above

bullet

Student's Transcripts (will be in withdrawal records from previous school

bullet

Student's Birth Certificate

bullet

Student's Immunization Record

bullet

Student's Social Security Card

bullet

Parent Picture ID

Withdrawal Process

The person who registered the student must be the one to withdraw him/her from school!

Withdrawing a student from LHS
bullet Parent/Guardian must come to the school with a picture ID
bullet Student must have each of his/her teachers sign a Withdraw Form (first signed by parent/guardian)
bullet All financial obligations must be paid before receiving a withdrawal packet

What is a Withdrawal Packet?
The following student documents are included in the packet

  1. Copy of transcripts (grades,  schools attended, test scores)
  2. Copy of Immunization Record
  3. Copy of Birth Certificate
  4. Copy of IEP, if applicable
  5. Copy of 504 plan, if applicable
