Lease, Settlement papers, Rental Agreement, or Most Current Property Tax
Bill) and a current bill (electric bill, telephone bill, or cable bill) |

Notarized statement if you are living with
someone, and proof of the person's residency as stated above |

Student's Transcripts (will be in
withdrawal records from previous school |

Student's Birth Certificate |

Student's Immunization Record |
 |
Student's Social Security Card |

The person who registered the student
must be the one to withdraw him/her from school! |

What is a Withdrawal
Packet?
The following student documents are included in the packet
- Copy of transcripts (grades, schools
attended, test scores)
- Copy of Immunization Record
- Copy of Birth Certificate
- Copy of IEP, if applicable
- Copy of 504 plan, if applicable
|

Withdrawing a student from LHS |
|
|
 |
Parent/Guardian must come to the
school with a picture ID |
 |
Student must have each of his/her teachers
sign a Withdraw Form (first signed by parent/guardian) |
 |
All financial obligations must be paid
before receiving a withdrawal packet
|
