One will be maintained in the office and the other will be maintained in the health room. These cards are extremely important and should be returned immediately to the school. Please be sure that both sides are completed. Also, be sure to sign the cards. The information provided is used by the school to reach parents in case of an emergency. It also provides information on where your child must go in case schools close early. It is important that these cards be kept up to date with your family information. Please notify the office if you have a change in information. |
