BE SURE TO SUBMIT THE FOLLOWING:

Proof of residency (must be an ORIGINAL lease or deed)
Current Utility bills/Government mail/Pay Stub (2 different pieces)(No cell phone bills)
Shots/Immunization records
Withdrawal/transfer papers from previous school
Name, address, telephone and fax numbers from previous school
Last Report Card/Current Grades
Parents/Guardians Government ID
Birth Certificate (Long Version)
Social Security Card
Notarized Letter (Obtain from Schools Main Office)
Two (2) peices of current mail, pay stub, bank statement, or bills with parent's name and address on them
IEP, 504 Plan, Functional Behavior Plan (if child is in Special Education)
