Address Changes
The change of address form is required.
In order to facilitate an address change, we will need
- The parent/guardian's Lease/Deed/Mortgage Statement/ Rental Agreement (whichever applicable)
- Two bills dated within the last 30 days from the date requesting the change showing the parent/guardian's name, the date, and the current address
- Valid ID for Parent/Guardian
Name Changes
Parent/Guardian must supply documentation to support the name change whether it be a divorce decree, marriage certificate, or other supporting documentation.
User Name, Password and Registration Questions
Parents/Guardians should contact the appropriate staff member below via email and include the student's first and last name and ID number.
