Registration Requirements

School Registration Requirements

Please note that all documents must be originals.  All mail must be delivered to your address through the USPS.  Electronic Statements will not be accepted.   

• Birth Certificate (showing Parents names)
• Registering Parent/Guardians valid government Issued photo ID
• Required Health Forms (Immunization Records, Health Inventory)
• Withdrawal Packet from previous school including:
  1. Current Report Card – including withdrawal grades
  2. Immunization Records (current and up to date)
  3. Copy of IEP or 504 Plan (if applicable)
  4. Copy of Birth Certificate        

Proof of Residence

If you own your home, you will need to provide one (1) of the following:

1. Deed (all pages with all signatures) with 1 current utility bill.
2. Settlement Papers (with all signatures) with 1 current utility bill.
3. Current Tax Bill with 1 current utility bill, or
4. Current Mortgage Statement with 1 current utility bill. 

If you rent your home, you will need to provide the following:

1. Lease/Rental Agreement (signed and dated by all parties).  Any lease NOT from a rental agency or realtor must be signed and notarized by both parties. PGCPS does not recognize sub-leases and all tenants must be listed on the lease agreement.
2. One (1) current utility bill (if a new resident, utility bill will be due within 30 days.

If you are living with a relative or friend, you will need a notarized Prince George’s County Public School Shared

Housing Form signed by both parties and must include the following:

1. The relative or friend with whom the parent(s) or guardian(s) reside must provide their proof of residence (see above bullets one or two), and
2. The parent or guardian must provide two (2) current proofs of your valid residence at the address used on the registration form (see below).  If you are new to the residence, you will be given thirty (30) days to provide the two (2) additional proofs of residence (below are acceptable forms)

• Wage Statement
• Current Bank Statement
• Medical Bill
• Credit Card Statement
• Auto Insurance/Registration

PRINCE GEORGE’S COUNTY PUBLIC SCHOOLS RESERVES THE RIGHT TO MONITOR THIS TYPE OF PROOF PERIODICALLY OR AS NEEDED (AS OFTEN AS EVERY 30 DAYS).

Registration is daily Monday - Friday from 8am - 2pm.
